2003 2WD 5-speed manual. The normal mpg from fueleconomy.gov is 20 city, 25 hwy.

Mind you, I currently have a cap on the back that rises 6 inches above the cab (it's a "wedge").

Does the Tacoma still have the belt drive fan? I have the last PU before the Tacoma (1994.5) and keep thinking about an electric fan.
I'm in Tucson so I need lots of Fan but if your in colder country you could save alot more with an electric fan.

If by belt drive you mean clutch fan that's bolted to the water pump then yes. My 2011 4cyl tacoma has one of those still... but not for long
