1. It's better to blip the throttle to approximately rev match. It will save wear on the synchros.
2. There's no single answer, aside from accelerating at or near optimal engine efficiency. Wide throttle increases engine efficiency, but if you go too far, the ECU will enrich the fuel:air mixture. A ScanGauge is helpful here.
