The basic issue is that the computer control module (ECM) will add fuel as the Oxygen sensor senses cleaner exhaust. As you know, the leaner a system burns the higher the NOX. I've seen these systems work on small engines (i.e. lawnmower engines) with no ECM, and run well. But place that same system into a vehicle that is x10 the displacement and has an ECM and it never works.
My brother owns a trucking company in OKC and he hired a company to retrofit his semi, but the only result was a clean exhaust. Without the HHO system the semi bellows black smoke but with the system there was no black smoke. But there was no mpg gains either. They adjusted the ECM (yes diesels have them too) but it isn't worth paying the large expense of the system just to clean up the exhaust with no payback. At the same time they took a smaller system and put it in his company pickup truck, Titan V8. Even with missing around with the ECM and tricking the mass air flow sensor, never saw a 1.10th of an mpg gain.
