When I would go to a car rental company in Europe, they would invariably hand over the keys to a stick-shift car, even knowing I was an American. I remember asking about an automatic so I could satisfy my curiosity, but was told such a car was considered a "luxury" car, and would be charged at luxury rates.
All they ever wanted to see was a valid driver's license and a passport. Oh, and a credit card, of course.
