Manifold vacuum is the enemy of efficiency, but when you eliminaute manifold vacuum you basically are at wide open throttle or close to it, which is why pulse and glide can increase mileage substantially.
If you restrict the incoming air to 50% of atmospheric pressure then you are in essence running the throttle wide open at 18,000 feet altitude, which is why turbo or superchargers are necessary on high altitude aircraft engines.
