It's more about stroke length and piston speed. Long stroke engines (hit and miss, ships) have extremely low crankshaft rpms yet the piston speeds are comparable to short stroke engines.

Also important is how much flywheel the engine has. The old two-cylinder John Deeres had lots of flywheel and long stroke; you could idle them slow enough to count the exhaust pulses!
