i think you're being OCD about the distributor position. if you have the ignition timing within spec, the objective has been met. whether the distributor is centered or not doesn't matter. and your mileage is good so that's a strong indicator that things are running relatively well.

the occasional miss at hot idle and lean running at hwy speeds could be a number of things other than ignition timing. for the hot idle issue, you can rule out the o2 sensor by simply unplugging it and driving like that for a while. if the miss at hot idle is gone, you've found the problem. if you replace the sensor and lean burn starts working properly, that's that.

also, when was the last time you replaced your cap, rotor, plugs, and wires?
