Too many unanswered questions.
That much condensation:
Head gasket
Thermostat
very short haul driving
Fouled plugs, apparently multiple times but no check enngine light!
Clean injectors, determine why it is running rich without CEL
Low coolant temperature, open loop?
Need more info.
