I don't bump-start all the time, but when I do, I try to be doing about 25-30mph, and bump start it in 5th. That way, when it does fire up, the revs don't climb really high. Also, as soon as the engine fires up, I disengage the clutch again, because at that point, I'm generally stopping anyway (if you saw my extremely low-traffic commute, this would make more sense).

Someone else here wrote up a much more elaborate, and probably better, bump-start technique, but I can't recall who that was.
