My wife has long since always unplugged the toaster when not using it, because she had one start on fire once. It usually ticks me off slightly, but after hearing about your experience, Ill be more thankful.
If you want to be hardcore safe, get an on/off plug-in timer with the on/off tabs that you can take out :

Plug-in Timers
(enlarge won't work)

Take out all the on/off tabs and only leave only one off tab.

When you make toast, you have :

- Spin the timer so that it is maybe an hour or less from the off tab.
- Turn the timer to the "on" position manually.
- Make toast as usual.

I know, I know, what a waste of time. But where there's toast/coffee/toaster oven paranoia, there's a way.
