They reassigned another client from a different therapist yesterday. I picked up two that way a couple of weeks ago and they tried with three before that, but for different reasons it didn't work out.

You likely have never received feedback from your boss because things are filtered both ways through your supervisor. Your supervisor is your boss and she is the person you need to please.
Your boss asks your supervisor “how is Xist doing” and she says something like: “He never fills out things correctly the first time, I always have to reject his stuff and make him do it correctly....”
