I've got a high threshold for danger because I'm very curious and have made a lot of mistakes (more likely to cause disaster, and more capable to handle it).

One time gas sloshed in the can just perfectly as I carried it to send a stream into my eye. My initial reaction was pain so intense I thought I was dying. I waited a second hoping death would come soon, but then realized I wasn't that lucky. I spent the next 5 minutes with my head in the bathtub with cold water pouring on my eye.

Anyhow, seems the guy made a series of poor decisions. That's usually what causes disaster; not a single unfortunate event, but a series of bad decisions.
