My wife is a biomedical engineer and used to design dental implants. What you have is called peri-implantits.

You "heli-coil" your jaw with a bone graph. You keep from losing another implant with excellent oral hygiene that includes daily flossing. Bone loss is caused by bacteria between the gum and tooth / implant generally from excess plague buildup.
(And this reminds me that I need to step up my flossing - as my wife and dentist continue to remind me on a regular basis)
