The innovation here is putting a twospeed transmission inside the motor rotor. Else the motor would have to be twice as heavy. On the downside it does require an hydraulic system for cooling, lube and shifting.

IIRC there's a 2 speed hub motor on Endless Sphere that works by reversing the motor direction for the 2nd gear.
