For sebacious stains, I find 1 cup of ammonia combined with detergent in every wash keeps ahead of it pretty well.

When the stain is old, it often has been 'set' in and may not come out. You've tried vinegar, you can try ammonia but not both at the same time. You could also try an oxygen-based bleach.
