Generally, it is modifying the exhaust so that it helps the engine produce more power or more torque. There are specific ways of doing that, including pipe length and diameter, the exact geometry of junctions of the pipes, what pipes are joined to what other pipes, and so on.
In general, most "tuned" exhausts out there are completely irrelevant to our needs. They are almost all optimized for high-RPM power, and many will sacrifice low-RPM torque, and reduce the efficiency of the engine in the RPMs where we typically drive.
