I remember speculating on this a few years ago and we talked about cotter pins. When I did research, it was hard to find one small enough to replace the pin without boring a bigger hole. This thread makes me also wonder if a cotter pin would be "tight" enough, if you know what I mean.

Assuming for the moment that it was small and strong enough, how about a pin that uses a snap ring (e ring) :

This would require special pliers, but that may be a good thing.
