The EPA website says "Based on 45% highway, 55% city driving". I checked one of their ratings, for a 2002 Subaru Legacy AWD, and it matches that: with their stated city/highway estimates, 45% highway gives their stated combined of 21mpg, while 55% highway gives 22mpg.
Maybe they changed it for the "new" ratings.
