I can tell you from first hand experience.
1998 Dodge Ram 5.9 4x4 Ex Cab Long Bed Tow Pig (Fathers truck)

Removed stock supposed 3" exhaust.
Factory muffler was over 3' in length, it was heavy enough the factory put a pair of HANDLES on it!

Opened up outlet of Catalytic Converter to fit a real 3" pipe (Not stock 2.5")
Went to a Single Flowmaster 50 Series (Old man likes it quite) 3" In/Out
3" Mandrel bent pipe exiting basically in the stock location.

Highway driving 80 MPH on hilly northern NM road, he saw about a 1-2 MPG increase.

Here's the kicker!

With trailer (26' Hi-Lo trailer) he went from and average (at ~65 mph) of about 10 to 14! Sure it's only 4 MPG, but think about it percentage wise!

What was effectively changed? THE EXHAUST! No headers, no high flow cats, just a decent 'cat back' system.
