OK, the few times I've had to start at or below -20°C, and couldn't plug in the coolant and oil pan heaters, it was more like:
  1. Wait a long time for the glow plugs to do their job,
  2. Hope that they left some juice in the battery for the starter,
  3. Turn the key,
  4. Wait for the idle to become less rough,
  5. Get the first signs of warm air after about 15-20km.

The -40 only showed up when the IR thermometer was pointed at something metal. The digital thermometer on our window showed the air temp was more like -27°C (-17°F).
