The overheating symptoms you described, 5000 RPM in second climbing a grade in heavy traffic are a good indication that the radiator is not flowing properly. A good radiator shop can check the radiator with a laser digital thermometer and determine if the radiator is truly the problem, but it could also be an air pocket in the cooling system that has not been properly bled. It could be other things but those two are the prime suspects, based on my experience.
