Around here so long as it goes and stops reliably, the horn and lights work and nothing falls off while the cop is watching, it's legal.
I have seen cops pull vehicles over for excessive smoking. The car, not the driver - too bad. But that's pretty uncommon now that everything is EFI.
We don't have annual inspections or emissions tests where I live.
