Looks like the washer stack is out of order. There are coned washers that mate up to allow angle adjustment but it seems those cones have flat washers between. The thick washer near the bolt head should be against the thickest washer closest to the pad, then the leaver then the flat washers and finally the bolt head. That way the pad surface can be nearly flat to the rim with just a bit of toe-in the stop the squealling.
