I think piston heat, lubrication (of both sets of rings), engine 'height' and the bottom piston sealing are the challenges in this design.

When you think of a 4-stroke per chamber its a bit of a mess:
power-compression; exhaust-power; intake-exhaust; compression-intake.
or
power-exhaust; exhaust-intake; intake-compression; compression-power.
Either way you 2 power strokes followed by 2 unpowered.
And where TH do you put the spark plug!? Valve size/clearance issues too.

For a Blown, like to OP posted, 2-stroke, with a common exhaust port; things look way better/simpler.
But you need to mix in oil again to lube the piston.
I also wonder about all that heat in the piston with no cool/ed surface
and exhaust port with no cool-down time.
