Watch the Diesel Brothers "reality" show, they build those. They take a cab from a wrecking yard and graft it to the vehicle to be expanded, and modify the frame or build a new one to lengthen it.

This is why we can't rely on technology to solve our problems, if we make engines more efficient, improve aerodynamics and decrease rolling resistance, people will just buy bigger cars.
We need a change in culture.

About 2% of new vehicle sales are electric and gas is $2 a gallon most places. It's a start since 10 years ago it was 0%.

Some do it, while others don't feel so comfortable to maneuver something bigger through city traffic. BTW one aspect that caught my attention is how new cars are usually not only longer, but also noticeably wider than an equivalent of 20 years ago.

I once saw a stretch Suburban for an extra-large family, but when I looked for it, all that I found was this confusing site: Six Door Conversions - Stretch My Truck

I am sure that the people doing the work can do math, but maybe the owners cannot tell the difference between 10.1 MPG and 9.5.
